Overview
Guava is a health data platform built to help DPC and concierge providers consolidate communications, labs, clinical records, wearables, and AI-assisted insights in one place, reducing the platform-switching that adds up across a busy panel.

By integrating Guava with Spruce, practices can:
- Search a specific patient's full message history directly from their Guava profile, going back years.
- Review communications alongside labs, wearable data, and visit records in one consolidated view, and use Guava AI to summarize conversations, flag recurring concerns, or draft responses based on the patient's full health context.
- Easily determine which patients haven’t been interacted with in a while based on Spruce conversations, visits, labs, and medical records, to improve patient engagement, retention, and outcomes.
Setup Instructions
To enable the Guava integration, follow these steps:

Enable API Access
In Spruce, send a message to the "Team Spruce" conversation and ask to enable API access to connect to Guava (this requires the Spruce "Communicator" plan).

Share Required Information
Once the Spruce team enables your API, go to the Integrations tab in Guava, select Spruce, and enter your API token and Access ID (copied from Spruce > Settings > Integrations & API > API Access).

Link Patients
Guava syncs conversations for patients who have been linked to their Spruce contact. Once patients are linked, you will see their messages in Guava, both in their profiles and in the Messages tab.
How It Works
Once enabled, the integration will allow practices to view Spruce messages without leaving the platform, search for messages for a given patient, and use the assistant to ask about conversations in messages.
Additional details about the integration:
- Message and SMS conversations are synced periodically
- Messages will appear in Guava under the Messages tab and in patients’ individual profiles in the Dashboard tab.
- Click “View in Spruce” to open the conversation in Spruce to respond directly.
Common use cases include:
- Searching a patient's full message history before a visit to quickly catch up on what’s been reported between appointments.
- Asking Guava AI to summarize the last three months of conversation with a patient, or to help draft a response to their most recent message.
- Reviewing a patient's messages alongside their labs and wearable data in one profile to connect what they've been reporting with what their health data shows.
Limitations
While the integration supports many key workflows, there are a few limitations to be aware of:
- Spruce notes, team chats, and internal notes are not currently synced to Guava
- Patients cannot see the messages in their Guava app yet
- The integration is available to providers on Spruce's Communicator plan with activated billing on Guava

Frequently Asked Questions
How do I know if the integration is working?
You should see linked patients and their past messages in Guava. If you don’t, check the Spruce integration settings in Guava or reach out to Guava and/or Spruce Support.
Can I customize what data is synced?
You can customize which patients’ messages are synced into Guava by choosing which patients to link during the integration setup process. You can always link new patients or remove linked patients by going to Guava’s Integrations tab > Spruce > Manage linked patients.
You can also choose which Spruce messaging links (aka URL endpoints) to sync to Guava. Keep in mind that all providers in your Guava organization will be able to see all conversations that get imported from these selected endpoints.
Is this integration secure and HIPAA-compliant?
Yes. Spruce and Guava Health comply with all relevant security and privacy standards, including HIPAA.
